Rotates a refresh token and mints the access token that goes with it.
Parameters
- token: string
  The refresh token presented by the app.
- tokens: AppTokenStore
  The token store opened for this request.
- runtime: IPortalRuntime
  Live portal runtime.
Returns Procedure<IGrantedTokens>
Procedure with the response body, or a failure naming invalid_grant.
